Pentucket Regional School District has partnered with Curative, a national biotech company, to provide a mobile vaccination clinic at the Middle School on Thursday, September 9th for the 1st dose and again on Thursday, September 30th for the 2nd dose. These will both be done during the school day from 8:30 AM to 11:30 AM. While parents/guardians do not need to be present at the clinic, they must complete the registration below if they want to have their child(ren) vaccinated.
For those wanting their Pentucket Middle or High school child(ren) vaccinated, the Pfizer vaccine will be administered to your child age 12+. The Pfizer vaccine is approved for emergency use authorization (EUA) by the FDA for individuals 12 and older.

How do I register and how long will it take?
· Registration is simple to complete using a computer or smart device such as a cell phone or tablet. It will only take 10 minutes to complete the process. You'll need to have general information and your health insurance card available to sign up. Note, health insurance is not required to participate.
· This process will only need to be done once.
